The Role of Community Doctor

Your doctor will decide when you are ready to go home. At this time, your family doctor or specialist in charge of your care will be contacted and given instructions regarding your care. We anticipate he/she will continue to be involved in your care. Correspondence will be ongoing between the transplant team and your doctor. A letter will be sent to your doctor after every clinic visit to notify him/her of your progress. After discharge, your doctor will send the transplant programme letters. The liver transplant coordinator will send forms to be filled in by your doctor to record your progress. This enables the transplant team to keep abreast of your progress.

When you arrive home it is VERY IMPORTANT that you:
Contact your own doctor/specialist and make an appointment – preferably within the first week.

Attend all medical appointments.

Have your blood work drawn as directed.
